Start Your Child’s Journey with NC InCK

Meet your care manager. You have the option to work with a care manager. They will help you meet your child’s health and wellbeing goals and gather a team to support your family time with your child and less time coordinating your child’s care.

Tell them what you need. With your team, you will create a plan for your child. This plan helps your child’s team members know your top goals to meet your child’s health needs.

Check in on progress. Meet with your care manager every three months to share your child’s progress. You can discuss any unmet or rising needs to support your child’s health and wellbeing.
