On April 22, 2024, UnitedHealth Group issued a press release, providing an update on the Change Healthcare cybersecurity incident that occurred on Feb. 21, 2024. Given the size of the data impacted, the investigation to determine whose data is impacted is expected to take several months. UnitedHealth Group believes this situation will impact “a substantial proportion of people in America” and is offering immediate credit monitoring and identity protection services, as well as a dedicated contact center to address questions. Healthy Activities Eligible for Rewards

Take healthy steps to earn My Health Pays® rewards. Members can earn up to $75 per year.

$10 Flu Shot
Ages 18+ years. Once per flu season: September through April.

$25 Adult Health Screening
Ages 20+ years. Once per year.

$15 Children's Well Visit
Ages 15-30 months. To earn $15 per visit, members may complete up to 6 visits between 0-15 months and up to 2 visits between 15-30 months.

$25 Children's Well Visit
Ages 3-21 years. Once per year.

$25 Children's Health Combo
Age 2. Requires completion of all of the following:

  • 4 diphtheria, tetanus, and acellular pertussis (DTaP)
  • 3 polio (IPV)
  • 1 measles, mumps and rubella (MMR)
  • 3 haemophilus influenza type B (HiB)

  • 1 chicken pox (VZV)

Ages 18+ years. To start, members must call the Quit Line at 1-866-QUIT-4-LIFE(1-866-784-8454).  Members must enroll in the program and complete 5 coaching sessions.

My Health Pays® Reward Dollars

Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays® Visa® Prepaid Card will be mailed to you.

Your My Health Pays® Visa Prepaid card is issued by The Bancorp Bank, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Card cannot be used everywhere Visa debit cards are accepted.

What if I have not received my My Health Pays® rewards card?

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have not received your card or you have questions, contact Member Services Monday-Friday, 8 a.m. to 6 p.m. EST.

How do I activate my My Health Pays® rewards Card?

Once you get your My Health Pays® rewards card in the mail, follow the instructions on the activation sticker. You will need to have your card with you to complete activation. 

Where can I check my My Health Pays® rewards balance?

Call 1-866-809-1091 to check your current My Health Pays® balance.

When do my My Health Pays® rewards expire?

Funds expire 90 days after your coverage ends or 365 days after the date reward was earned, whichever comes first.
