Earn My Health Pays® rewards when you complete healthy activities like a yearly wellness exam, annual screenings, tests, and other ways to protect your health.
Use your My Health Pays® rewards to help pay for:

- Shop at Walmart for everyday items
**This card cannot be used to buy alcohol, tobacco, or firearms products.
Healthy Activities Eligible for Rewards
Take healthy steps to earn My Health Pays® rewards.
Members can earn up to $75 per year.
$10 Flu Shot
- Ages 18 and over
- Once per flu season: September through April
$25 Adult Health Screening
- Ages 20 and over
- Once per year
$25 Cervical Cancer Screening
- Ages 21-64
- Once per year
$25 Diabetic HbA1c Lab Test
- Ages 18-75
- Once per year
$25 Tobacco Cessation
- Ages 18 and older
- To start, members must call the Quit Line at 1-866-QUIT-4-LIFE (1-866-784-8454)
- Members must enroll in the program and complete 5 coaching sessions
$25 Behavioral Health
- Ages 6 and older
- To earn go to a behavioral health provider within 30 days after a behavioral health hospital stay
- Ages 1-17
- To earn go to a counseling appointment within 30 days after anti-psychotic prescription
$25 Children's Well Visit
- Ages 0-15 months
- To earn $25, complete all well-child visits per well-child checkup schedule.
- Members may complete up to six (6) visits between 0-15 months.
- Ages 15-30 months
- To earn $25, complete all well-child visits per well-child checkup schedule.
- Members may complete up to two (2) visits between 15-30 months
- Ages 3-21 years
- Once per year
Healthy Pregnancy Rewards
- $25 Prenatal Care Visits*

- Members who complete a prenatal visit* can choose one (1) of the rewards listed:
- Choice of stroller, portable playpen, car seat or six (6) packs of diapers

*Visit must be with an OB/GYN, other prenatal care practitioner, or PCP.

- Two (2) flu vaccines by second birthday
$25 Immunizations for Adolescents
- Ages birth to 13. Must complete all by 13th birthday:
- One dose of meningococcal vaccine
- One tetanus
- Human papillomavirus (HPV) vaccine series
- Diphtheria toxoids and acellular pertussis (Tdap) vaccine
$25 Children's Health Combo
- Age 2
- Requires the completion of all the following:
- Three polio (IPV)
- One measles, mumps and rubella (MMR)
- Three haemophilus influenza type B (HiB)
- Three hepatitis B (HepB)
- One chicken pox (VZV)
- Four pneumococcal conjugate (PCV)
- One hepatitis A (HepA)
- Two or three rotavirus (RV)
- Two influenza (flu) vaccines
- Four diphtheria, tetanus and acellular pertussis (DTaP)
My Health Pays® Reward Dollars
Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. Eligible members receive rewards based on paid claims. Eligible members are those with Centene as the primary payer. If you are earning your first reward, your My Health Pays® Visa® Prepaid Card will be mailed to you.
Your My Health Pays® Visa Prepaid card is issued by The Bancorp Bank, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Card cannot be used everywhere Visa debit cards are accepted.

Frequently Asked Questions
Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have not received your card or you have questions, contact Member Services Monday-Friday, 8 a.m. to 6 p.m. EST.
Once you get your My Health Pays® rewards card in the mail, follow the instructions on the activation sticker. You will need to have your card with you to complete activation.

Funds expire 90 days after your coverage ends or 365 days after the date reward was earned, whichever comes first.
