PCP Change Requests and Member Moves – Effective February 15, 2026

Dear Provider,

This notification serves as important action information regarding Primary Care Provider (PCP) changes and member move requests in preparation for the upcoming health plan merger.

Carolina Complete Health (CCH) and WellCare of North Carolina will unite as a single statewide Provider-Led Entity effective April 1, 2026. To support a smooth and accurate transition, the following guidance must be followed:

Important Dates and Requirements:

  • Effective February 15, 2026: Providers should no longer submit PCP change requests or member move requests to WellCare of North Carolina.
  • Requests submitted after February 15, 2026, will not be processed due to the State’s enrollment update process and the upcoming Unified Plan go-live.
  • All PCP change requests must be submitted to Carolina Complete Health moving forward. Please use the following link to access Panel Management forms: Manuals, Forms, and Guides >Provider Forms> Panel Management (PCP Changes and Reassignment Request Forms)

Action Required:

Please ensure your administrative and care coordination teams are aware of this change and submit all future PCP change requests in accordance with the guidance above to avoid processing delays.
