Notice of Funding Availability
Grant Proposals will be accepted beginning: January 1, 2025
Purpose: WellCare of North Carolina is pleased to announce the availability of grant funds focused on improving health equity and social determinants of health in the community.
At WellCare, we’re committed to improving community health and well-being by addressing key social factors that impact people’s lives. In North Carolina, health disparities persist because of challenges like income, education, support networks, healthcare access, and neighborhood conditions. To make a real difference, we aim to partner with local nonprofit organizations committed to creating lasting, sustainable change. Community based organizations are eligible to apply for funding for programs that will address one of the primary focus areas.
Housing: Initiatives related to affordable housing, homelessness prevention, or housing stability programs.
Food: Programs aimed at improving access to nutritious food, reducing food insecurity, or promoting healthy eating habits.
Interpersonal Safety: Projects focused on violence prevention, domestic abuse intervention, or community safety initiatives.
Transportation: Efforts to enhance transportation access for healthcare appointments, employment opportunities, or community services.
Employment: Programs supporting job training,
workforce development, or employment placement services.
Health Equity: Outreach initiatives addressing the needs of historically marginalized populations. Cultural competency or implicit bias training for healthcare or community service providers.
Other Factors: Innovative approaches targeting additional social determinants not listed above.

Funding Availability
Applicants may request a minimum of $20,000 and maximum $150,000 for 12 months of funding.
Proposed Project Period or Contract Term:
July 1, 2025 – June 30, 2026
Eligibility:
All community-based organizations that provide direct programs to the local populations are eligible to apply. While non-profit status is preferred, all eligible organizations are encouraged to apply.
As a condition of receiving a grant award, successful applicants must agree to:
- Submit performance reports quarterly throughout the grant term
- Document data on number of individuals and families served by applicable program.
- Participate in public relations campaign to announce the grant award.
